• Family Health Worker - OB/GYN

    Charles B. Wang Community Health Center (Queens, NY)


    + Coordinate quality assurance activities related to women’s health services and projects

    + Responsible for case management in general prenatal services, preventative health screening, and abnormal screening results

    + Ensure comprehensive services are provided to patients by conducting chart reviews and reviewing intake forms

    + Responsible for data entry of intake forms, surveys and questionnaires, and database management

    + Work closely with directors, unit managers and staff on clinical matters, including clinical staff training

    + Work closely with Health Center departments and outside agencies on matters pertaining to clinical systems

    + Liaison with community agencies related to women’s health projects

    + Assist in completing administrative reports, compiling statistics on program activities, and keeping track of all completed activates as part of programs review

    + Perform other duties as assigned
