             Program Supervisor-Adult Group Homes Hillcrest Family Services 1 Positions ID: 537390852 Posted On 09/22/2025 Refreshed On 10/23/2025 Job Overview Responsibilities The **Program Supervisor** for the **Adult Group Homes** will oversee the daily operations, ensure the implementation of individualized care plans, and maintain a supportive and therapeutic environment for residents with mental health needs. This role involves managing staff, coordinating with healthcare providers, and ensuring compliance with regulatory standards. In addition, you will: + SuperviseandprovidecoachingtoDirectSupportProfessionalsinAdultGroupHomes. + AssistProgramManagerinthereferral,intake,anddischargeprocessesfortheprogramandmonitorscompletionof all requirements. + MaintainregularcontactwithCentralizedAdmissionsinallaspectsofthereferral,funding,andadmissionprocess, including providing updates to referral sources regarding upcoming openings. + Setupinitialmeetingwithclient,IHHandothersinvolvedwiththeclienttodevelopatreatment plananddiscussthe needed funding with IHH. + Operate theprogram(s)peragency,licensing,andregulatorystandards(i.e.HFS,DIAandJointCommission). + Monitorthecompletionandtimelinessofallclientserviceplans,staffingprogressreportsandotherclinicalfile requirements. Reports staff performance issues regarding this to the Program Manager. + Verify thatcontracting/fundingobligationsaremetforeachclientandreportsanyissuestotheProgramManager. + Ensurestaffareawareofmodelsofclinicalinterventionusedwithintheprogramandthattheycanimplementthe techniques effectively and appropriately to provide for a therapeutic environment. + Conductassessmentsandcreatesserviceplansbasedonclients’needs,wants andabilities.Providescase coordination for clients. + Effectivelycommunicate andcoordinateserviceswithothercommunitysupportpersonsonaregularbasis. Collaborates effectively, as appropriate, with other programs within Hillcrest or with external entities. + Facilitate anddocumentregularteammeetingsandmonthlyin-services,includingmakingsuresevereweatherand fire drills are completed and well documented. + Maintaindailyscheduletoensureappropriatestaffinglevelsaremet.Participatesinshiftcoverage–including holidays, weekends, and on-call crisis resolution. + Otherdutiesasassigned. Requirements Education: + Bachelor's degree is a human services field or related field is required. Experience: + Prior case management experience is preferred. + Prior supervisory experience is required. + Prior experience working with adults with chronic mental health challenges is preferred. + Prior experience with health care documentation and electronic health records (EHR) is preferred. Knowledge/Skills: + Strong leadership and management skills. + Excellent verbal and written communication skills. + Knowledge of mental health disorders and treatment modalities. + Strong organizational and problem-solving skills. + Ability to handle sensitive and confidential information with discretion. Licenses/Certifications: + Current and valid driver's license and ability to meet agency driving requirements is required.
