Job Description:
The Home Dialysis Program Manager has both leadership and clinical responsibilities. This role is accountable for implementing and adhering to all system best practice standards. This role ensures that home dialysis within Kidney Services achieves and sustains excellence in patient safety, patient experience, quality, access, innovation and stewardship. The Home Dialysis Program Manager typically has the following accountabilities: Leadership of all home dialysis programs and home dialysis staff to include home dialysis nurses and technicians. Is a champion for home dialysis and oversees the education of patients and staff members to ensure competent and safe transitions of care within the home dialysis setting.
This position is a system-wide leadership role that facilitates and coordinates the home dialysis program operations related to specific development strategies, team development, planning, implementing, evaluating and compliance initiatives. The Home Dialysis Manager will take the lead when it comes to new innovations in the arena of home dialysis and will drive growth and access to home dialysis for all patients.
Essential Functions
+ Approves, creates, and supervises new programs in collaboration with appropriate stakeholders.
+ Participates in creating and implementing the program vision, direction, and continuous improvement process, managing patient care teams, and providing direct patient care.
+ Identifies staffing needs and oversees recruiting, hiring, terminating, disciplinary action, and scheduling to maintain adequate services to ensure both individual and team productivity. Provides performance management and conducts annual performance reviews. Leads the orientation and training of new employees, provides continuing education and in-services to staff.
+ May be responsible for or assist with managing the operating budget, supplies and equipment.
+ Maintains current knowledge of standard practices for discipline. Ensures that practice is compliant with regulations and that changes in practice are communicated appropriately, and changes are made in a timely manner.
+ Identifies alternative, innovative means of providing home dialysis care to increase efficiency and effectiveness.
+ Is responsible to participate in the development of policy, procedure and guideline for Kidney Servies and ensure staff follow established policy, procedure and guidelines to improve patient care, improve efficient use of resources, and optimize patient outcomes.
+ Oversees caregivers to ensure documentation for each patient is done in a complete, thorough, appropriate, and timely manner, according to regulatory, hospital, department, and team requirements.
+ Understands the billing aspects of home dialysis and functions as a resource for the home team when it comes to proper billing practices in the home dialysis setting.
+ Promotes appropriate and consistent interaction and communication among team members and other professionals. Promotes effective working relationships among staff to facilitate achieving goals and objectives.

Physical Requirements:
Qualifications - Required
+ Bachelor’s degree. Education must be obtained from an accredited institution. Degree will be verified.
+ Obtain Basic Life Support (BLS) for healthcare providers prior to start date.
+ Experience in a role requiring effective verbal, written and interpersonal communication skills.
+ Ability to travel through a specified geography to meet with facility leaders and employees to accomplish work.
Qualifications - Preferred
+ Master’s Degree in related field (Nursing, Business, Healthcare, etc.)
+ Home Dialysis experience
+ Experience in a role requiring project management skills, including the demonstrated ability to conceptualize, develop and implement projects on time and within budget constraints.
+ Experience in a leadership role involving teams and specialized projects.
+ Experience developing policies and procedures.
+ Experience working in an integrated healthcare system.
+ Experience analyzing data.
+ Experience managing dialysis operations for a full-service dialysis center within a health care system environment.
Physical Requirements
+ Ability to evaluate and interpret color coded test results on test strips.
+ Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
+ Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
+ Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
+ Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
+ Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.).
+ May be expected to stand in a stationary position for an extended period of time.
+ For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$50.97 - $78.69
