7pm-7am
To oversee all daily admissions and bed management within UnityPoint Health – St. Luke’s. The aim is to balance the access demands of the Emergency department and those of urgent/non-urgent elective work with available beds. The Patient Flow Coordinator will provide the clinical lead for a team that will include schedulers, validation and administrative personnel. The aim of this team is to optimize the continuum of care by managing the patient effectively throughout the whole of their care episode, from admission into the hospital and back. The process of enhancing patient management is accomplished by and safe admission/placement of patients entering the hospital and those who are transferring from the Critical Care areas, or within units of the hospital. Placing patients in the right bed efficiently as possible.

Responsibilities
Clinical Excellence/Leadership/Patient Care
Demonstrates competence in the skills, (processes, procedures, and equipment) necessary to carry out assigned duties as identified below. Demonstrated competence in recognizing and responding to patients physical, mental, emotional, and developmental needs should be evaluated in relation to the patient populations served as appropriate for each responsibility.
Patient Care
+ Maintains a safe and clean environment.
+ Performs prescribed Unit Safety Checks.
+ Interprets physician's orders.
+ Assists with facilitating patient procedures, admissions, discharges and transfers.
+ Promotes an environment of medication safety.
+ Monitors patient sensitive indicators (real time) and collaborates with manager/director for unit improvements.
+ Reports/monitors unit events per policy.
Technical
+ Is knowledgeable of equipment and supplies.
+ Reports equipment problems or malfunctions.
Managerial
+ Coordinates services and activities of Unit
+ Guides, directs, and delegates responsibilities and supervises unit personnel in the delivery of health care.
+ Provides leadership and nursing expertise.
+ Supervises unit to ensure appropriate levels of staffing, equipment and supplies.
+ Monitors changes in policies/practices performed and in conjunction with Manager. Integrates the data into an overall planning process for the Department.
+ Involved in development of patient plan of care which includes patient assessment and cooperation with staff and physician.
Primary Nurse Responsibilities
+ Assesses the needs of patients and families, providing appropriate teaching and making the necessary referrals to provide continuity of care.
+ Develops an individual plan of care using a collaborative approach with the patient/family and health care team.
+ Promptly completes documentation to assure an accurate legal record of patient’s care.
+ Delivers high quality care utilizing the nursing process while demonstrating knowledge and clinical skills of the specialty required on assigned unit in accordance with established policies, procedures and standards of care.
+ Utilizes holistic approach (considering biopsychosocial, spiritual, educational, safety, and related criteria) appropriate to the age of the patients and families served in the assigned service area.
+ Delegates and provides direction for other healthcare providers and hold them accountable for their involvement in the plan of care.
+ Utilizes critical thinking in making independent judgments, and maintains responsibility and accountability for the knowledge of conditions of assigned patients.
Leadership Education Responsibilities
Provides learning opportunities to health care associates.
Ensures employees receive necessary training and orientation to area.
Demonstrates leadership abilities in provision of Nursing Care.
+ Participates at staff meetings, in-services, and/or committee meetings.
+ Integrates role as patient advocate into professional practice.

Primary Nurse Responsibilities
+ Continues professional development and involvement in professional activities that enhance nursing practice by sharing knowledge through networking and research.
+ Assess and chooses teaching strategies appropriate to the patient’s and their families’ age specific needs, culture, gender, readiness to learn and level of comprehension.
+ Consults appropriate resources within the hospital to provide specialized education.
+ Demonstrates knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.
Qualifications
+ Education: Graduate of accredited nursing program required. BSN or MSN preferred.
+ Experience: Two (2) years of hospital critical care experience and a proven history of positive leadership skills in an active hospital leadership role or lead nurse or charge nurse role required.
+ Positive history of collaboration, team work, best practice, and role model of FOCUS values.
+ Licenses/Certifications: Current licensure in good standing to practice as a Registered Nurse in Iowa. Must possess and maintain current Basic Life Support (BLS) certification prior to hire. Must obtain and maintain Advanced Cardiac Life Support, Pediatric Advanced Life Support within 6 months of hire. Trauma Nurse Core Course within one year of hire. Must obtain NIHSS education within 90 days of hire and maintain every two years thereafter. Completion of four hours of stroke education prior to the end of orientation and annually. Valid driver’s license when driving any vehicle for work-related reasons.
+ Knowledge/Skills/Abilities: Writes, reads, comprehends and speaks fluent English. Multicultural sensitivity. Microsoft Office – basic computer skills. Customer/patient focused. Critical thinking skills using independent judgment in making decisions.
+ Other: Use of usual and customary equipment used to perform essential functions of the position. Work may occasionally require travel to other UPH facilities. May drive a UPH vehicle, rental or own vehicle.
