+ **Home visits in assigned service area:** Pasco surrounding areas

The **Home Health Field LPN** provides **hands-on care** in the home for a group of patients consistently using the nursing process and clinical standards and protocols. **Responsibilities include** :

+ Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse

+ Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse

+ Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status

+ Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Home Health - LPN - Requirements:

+ BLS (Basic Life Support)

+ LPN (Registered Nurse) License

+ Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

+ Required Associate's or Diploma Nursing

+ Required 1 year Licensed Practical Nurse

+ _Preferred_ 1 year Home Care
