Work Schedule: Days and Evenings with Call Weekend and Holiday Rotation

Hours: Sunday: 10am-2pm. Monday-Friday: 8:00am-8:30pm. Saturday: 8:30am-2:30pm. Rotating weekends (every 4 to 5); rotating on-call

Summary:

In conjunction with the Director of Pharmacy and Supervisor, Pharmacy, ensures compliance with the rules and regulations that govern the practice of Pharmacy primarily servicing Long Term Care, Hospice Inpatient Unit, and Life (PACE) patients.

Responsibilities:

EDUCATION

+ Graduate from an accredited college of Pharmacy required.

+ Licensed in New York State required

+ Pharmacist Doctorate (Pharm D) preferred

+ Current registration in New York State required

EXPERIENCE

+ Demonstrated experience in program and service development,

+ Quality assurance and medication use indicator experience required

+ Demonstrated experience in cost containment programs

KNOWLEDGE, SKILL AND ABILITY

+ Ability to communicate effectively.

+ Ability to operate compounding devices, fax machine, copy machine, prescription balance, Pharmacy computer system,

+ Strong Supervisory skills.

+ Good manual dexterity.

+ Strong math skills.

+ Strong written and verbal skills

+ Long Term Care or Retail experience preferred.

+ Computer skills

+ Pharmacy compounding, packaging, as applicable
