    6:00am-5:30pm (7on/7off)

     

    Novant Health is seeking a Clinical Pharmacist-IV Room 7on/7off to join our acute care pharmacy team, to deliver optimal medication management across all dimensions of care, every time.

    IV Room and/or Chemo Compounding and Verification experience, preferred.

    Essential functions include:

    + Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standards

    + Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems

    + Checks and verifies all IV room compounded products by technicians

    + Provides second check and backup support for chemo orders and verification

    + Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care

    + Makes appropriate evidence-based, patient- centered medication recommendations

    + Participates in the management of medical emergencies

    + Provides discharge medication review, reconciliation, and counseling as appropriate

    + Maintains proficiency in and actively engages in central dispensing functions and operational programs

    + Oversees pharmacy operations and technician practice/activities

    + Maintains proficiency in preceptor roles and actively emulates education/ mentorship skills

    What We're Looking For

     

    + Education: 4 Year / Bachelors Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification, required. PharmD, preferred.

    + Experience:

    + Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy. Oncology experience preferred for oncology roles, required.

    + For Infectious Diseases/Antimicrobial Stewardship roles: ASHP - accredited PGY2 Infectious Disease residency OR minimum of 2 years of equivalent clinical pharmacy experience in infectious diseases/antimicrobial stewardship, required.

    + Licensure/Certification:

    + Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required.

    + Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role, required.

    + Additional Skills Required: Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
