Job Summary:
As the Pediatric Primary Care Social Worker, you provide care management for high-risk pediatric patients and family members/caregivers managing psychosocially complex conditions.
You will provide social work and care management services to pediatric patients and caregivers to promote understanding and resolution of problems.

REQUIREMENTS
+ Education:Master’s Degree in Social Work
+ Skills:Exceptional problem-solving, critical thinking, organizational, written/verbal communication skills, and high computer literacy. Ability to work in a self-directed environment with attention to detail and follow-through.
+ License: Active LISW license
PREFERRED REQUIREMENTS
+ Years of Experience: Experience in a medical or other multi-disciplinary agency is desirable, and experience that matches the assigned practice specialty.
