    Coordinates and provides the pharmaceutical care of patients for an assigned clinical unit and participates in patient care rounds with medical staff personnel.

     

    Cleveland Clinic is comprised of a 1,400-bed academic medical center and 17 additional hospitals, including 5 in Southeast Florida. In total, there are nearly 6,000 inpatient beds and over 1,400 pharmacy employees, more than 300 physician practices, 20 ambulatory pharmacies, significant inpatient and outpatient clinical services and a pharmacy budget exceeding $1 billion.

     

    A caregiver in this role will provide advanced clinical pharmacy services to support patients with endocrine disorders through collaborative practice agreements. This caregiver will work closely with endocrinologists, primary care providers, diabetes educators, and interdisciplinary care teams to optimize medication therapy, enhance disease management, and improve patient outcomes. Responsibilities include initiating, adjusting, and discontinuing medications under consult agreements, conducting individual and group patient visits (in-person and virtual), interpreting continuous glucose monitoring data, managing insulin pumps, and delivering patient and provider education. The pharmacist may also support pharmacy services in specialty clinics, including post-kidney transplant and diabetes in pregnancy.

    A caregiver in this role will work full time, Monday - Friday from 8am-5pm.

    A caregiver who excels in this role will:

    + Coordinate and provide pharmaceutical care for patients.

    + Participate in patient care rounds.

    + Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients.

    + Provide general and patient-specific drug information.

    + Educate clinical pharmacists, technicians, pharmacy students, post-graduate trainees, allied health professionals, nurses and physicians in pharmacotherapy.

    Minimum qualifications for the ideal future caregiver include:

    + PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy

    + If hired for a specialty area where specialty residency training is available, then specialty residency is required

    + Current state licensure as a Registered Pharmacist (RPh) OR eligible for reciprocation in the current state within 90 days of hire

    + Completion of a post-graduate year 2 residency (PGY2) or fellowship OR completion of a post-graduate year 1 residency (PGY-1) and two years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmaceutical monitoring OR four years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmacotherapeutic monitoring

    + For individuals required to administer immunizations: current Basic Life Support (BLS) Certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire

    + For individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA) or certification obtained within 1 year of hire

    **Physical Requirements** :

    + Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.

    + Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.

    + Position often includes reaching, stretching, stooping, climbing steps and sitting.

    + Must have normal or corrected vision.

    + Must have ability to clearly communicate verbally by phone or in person

    **Personal Protective Equipment** :

    + Follows standard precautions using personal protective equipment as required

     

    Pay Range

     

    Minimum Annual Salary: $101,900.00

     

    Maximum Annual Salary: $155,390.00

     

    The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate's actual compensation will be determined after taking factors into consideration such as the candidate's work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.).
