UPMC Family Hospice IP Aide

Provide designated care under the direction of the primary service provider in the hospice inpatient unit (IPU) to achieve identified goals of patient care.

Responsibilities:

+ Participate in all required agency meetings.

+ Identify, develop and participate in process improvement opportunities within the home health agency that will enhance the quality of services we provide.

+ Communicate appropriately with the interdisciplinary IPU care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.

+ Provide safe and direct personal care to patients in the IPU setting under direct supervision of appropriate professional

+ Fulfill agency responsibilities by completing all required forms accurately and in a timely manner.

+ High School Diploma or GED - Required

+ Must have successfully completed a training and competency evaluation program as specified in Home Health and/or Hospice as applicable: Condition of participation:

+ Home Health aide services. One (1) year experience - Preferred

+ Certified Nurse Aide -Preferred

+ Must maintain annual competency evaluation scores as determined by regulatory requirements.Licensure, Certifications, and Clearances:Required: CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

+ Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

+ Act 33

+ Act 34
