Job Overview
Provides for the development, coordination, implementation, and evaluation of a regional stroke program across a care continuum. Works collaboratively with multi-disciplinary team, Neuroscience Service Line Cerebrovascular Services leadership team, and other domain members to achieve consistent excellent customer experience and program initiatives.
Responsibilities
+ Program Coordination:
+ Demonstrates expert knowledge of stroke patients.
+ Demonstrates expert knowledge of current AHA Guidelines as well as emerging science and evidence based practice into Stroke program standards.
+ Supports the development, implementation, review, and revision of Stroke Program standards, policies, procedures, and protocols.
+ Assists in coordination of the Telestroke Program.
+ Develops and utilizes materials that reflect knowledge and understanding of impact of stroke on patients and the stroke recovery process and benefits of early intervention.
+ Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served
+ Teamwork/Collaboration:
+ Acts as a liaison with community and other Fairview entities.
+ Works collaboratively with a variety of community resources and Fairview entity representatives.
+ Facilitates participation in internal and external research opportunities related to stroke.
+ Certification:
+ Lead/Co-lead Comprehensive Stroke Center, Primary Stroke Center, and Acute Stroke Ready Hospital certification processes.
+ Stroke Program Quality-Performance Improvement:
+ Assume day-to-day responsibilities of stroke process and performance improvement activities as they relate to nursing, and ancillary personnel in addition to working collaboratively with Stroke Program leadership to carry out these same functions for all staff, including provider staff.
+ Oversight of stroke data abstraction, analysis, and reporting.
+ Analyze fall out from Stroke data and manage critical pathway time frames. Evaluates patient outcomes and identifies and intervenes on variances.
+ Participate in meetings and initiatives related to improving quality of stroke patient care. Share quality data as needed, including lead quality committee and Emergency Department meetings. Participate in statewide quality projects
+ Education:
+ Plans, coordinates and evaluates stroke education for internal and external audiences.
+ Collaborates with the Professional Practice and Education team, participates in planning, coordination, and evaluation of stroke education for nursing.
+ Collaborates with medical leadership in planning, coordination, and evaluation of stroke education for residents and providers related to the care and management of a stroke patient.
+ Provides stroke related education to external audiences, including planning and participating in educational programs targeting, EMS providers in the local and regional communities.
+ Participate in educational opportunities appropriate to professional goals.
Required Qualifications
+ Bachelors Degree in Nursing
+ 3 years experience inpatient or outpatient neuroscience patient care
+ Registered Nurse Active issued by Minnesota Board of Nursing
+ SCRN or CNRN certification within two years of hire
Preferred Qualifications
+ 5 years experience inpatient or outpatient neuroscience, emergency department or critical care patient care

Compensation Disclaimer
The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Hiring at the maximum of the range is not typical.
