    Key Responsibilities

    + Deliver Remarkable Patient Experience:

    + Collaborate with healthcare teams to support personalized patient relationships.

    + Assess, plan, implement, and evaluate individualized care plans promoting optimal health.

    + Accept responsibility and accountability for medication management and patient care.

    + Leadership:

    + Serve as a pharmacy team leader and change agent to improve clinical and financial outcomes.

    + Build collaborative partnerships with stakeholders and exemplify Novant Health’s mission and values.

    + Participate in process improvement initiatives and demonstrate leadership skills like delegation and problem-solving.

    + Patient Care & Medication Safety

    + Responsible for optimal medication management and promotion of medication safety.

    + Participate in direct patient care activities and rounding opportunities where available and in alignment with department and organizational goals.

    + Monitor therapy for appropriateness, dosage, effectiveness, interactions, and adverse drug reactions. Perform patient assessments. Review and act upon clinical decision support surveillance.

    + Complete organizational and/or departmental required documentation.

    + Participate in patient education and wellness-based initiatives as driven by scope of practice. Prescribe and/or administer medications as allowed by law and scope of practice.

    + Maintain up to date, evidence-based knowledge required for services provided.

    + Review and process orders specific to the population served.

    + Supervise, check, and control quality and accuracy of work performed by pharmacy team members.

    + Prioritize workflow ensuring all services are performed in a timely manner. Maintain and follow formulary approvals, interchanges, stocked medications and non-formulary process.

    + Quality Improvement & Compliance:

    + Lead or participate in medication safety and quality improvement projects.

    + Develop and review clinical protocols, policies, and order sets.

    + Report performance metrics and support formulary management.

    + Develop and implement drug shortage response plans.

    + Support compliance with regulatory standards and Novant Health policies.

    What We're Looking For

     

    + Education: 4 Year / Bachelors Degree, required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification. PhD, preferred. Doctorate of Pharmacy.

    + Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist. Outpatient or retail pharmacy preferred, required.

    + Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Basic Life Support Certification, preferred.

    + Additional Skills/Requirements (required): Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
