Job Title: Raise the Age Care Coordinator Department: Raise the Age Care Coordination
Position Summary: Coordinate services for families enrolled in the Raise the Age Care Coordination program, integrating the Wraparound Model, & Restorative Justice Practices.

Compensation
Bachelor's Degree: $21.30 - $23.38 per hour
Master's Degree: $23.10 - $25.18 per hour

Position Specific Duties/Responsibilities:
+ Work collaboratively with youth, families and Erie County staff to identify strengths and needs and develop, implement and evaluate an individualized service plan that addresses the social determinants contributing to the behaviors identified, with a goal to realign with prosocial behaviors.
+ Work closely with youth and families to build a Child Family Team (CFT) of supportive people that include natural, community and system supports that will help the youth repair and reconnect with the community and achieve their short and long term goals.
+ Provide in-home and community based interventions, education, and mentoring that assist youth to develop positive relationships, independent living skills, and allows youth to meet expectation set by their parents/caregivers, their community, Erie County and the legal system. Additionally, provide advocacy, case management, and crisis intervention services, including regular telephone, face to face contact, and availability by cell phone.
+ Refer and link youth to culturally competent , community based, formal and informal supports that address needs identified in the assessment process and help youth meet their established goals and the expectations of Erie County and the legal system.
+ Participate in planning and the facilitation of twice monthly youth groups and Restorative Justice Circles.
+ Ensure accurate, thorough, and timely completion and filing of all necessary documentation in TenEleven and/or CONNECTIONS according to program and contractual expectations.
+ Attend required meetings and trainings offered by Gateway-Longview and Erie County Probation / Department of Social Services
+ Other duties as assigned
Knowledge, Skills, and Abilities
+ Must have excellent communication and customer service skills
+ Ability to multi-task and be detail oriented
+ Ability to work independently and be an enthusiastic team player
+ Possess assessment skills, ability to engage families, and establish and maintain relationships with social services and other service providers
+ Must have a high degree of discretion dealing with confidential information
+ Ability to work effectively with clients, staff, families, and community contacts
Qualifications
+ Bachelor’s Degree in a human services related field with 4 years of experience
+ Master’s Degree in a human services related field with 2 years of experience
+ Experience providing direct care services or linkage services to at risk youth/children and families preferable within the juvenile justice / delinquency field
+ Valid New York State driver’s license, adequate auto insurance, and reliable transportation to travel locally on a regular basis is required
