Job Title: Case Manager (QIDP Qualified)

About the Role
We are seeking a dedicated Case Manager to oversee individualized service planning and ensure high-quality care for individuals with intellectual disabilities. This role combines case management, compliance oversight, financial auditing, staff development, and advocacy to deliver exceptional support services.
Key Responsibilities
Case Management
+ Manage caseload and oversee development, implementation, and review of Overall Service Plans (OSP).
+ Conduct assessments, facilitate team meetings, and maintain accurate documentation.
+ Ensure compliance with licensing, certification, and accreditation standards.
Quality & Compliance
+ Perform monthly on-site observations and quality reviews.
+ Monitor safety standards and program delivery.
Financial Oversight
+ Audit financial files quarterly.
+ Ensure individuals access entitled benefits and meet reporting requirements.
Personnel & Training
+ Assist with hiring Direct Support Professionals.
+ Provide OSP training and identify ongoing staff development needs.
+ Maintain CPR/First Aid Instructor certification.
Communication & Advocacy
+ Maintain open communication with individuals, families, and team members.
+ Participate in committees and investigations as needed.
+ Adhere to on-call rotation.
Qualifications
Minimum Requirements
+ Bachelor’s degree in a related field.
+ 1 year of experience working with individuals with intellectual disabilities (QIDP qualified).
+ Strong communication, organizational, and time management skills.
+ Knowledge of OSP process and behavioral supports.
Preferred
+ 2+ years of professional experience in human services.
+ Familiarity with habilitation practices and ICF/ID standards.
Job Type & Location
This is a Permanent position based out of Fargo, ND.
Pay and Benefits
The pay range for this position is $50000.00 - $60000.00/yr.
