We are seeking a dedicated and skilled Advanced Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care, perform diagnostic evaluations, and collaborate with physicians and other healthcare professionals to ensure comprehensive treatment plans. This role requires a strong background in internal medicine and the ability to manage various patient needs effectively.

Responsibilities

+ Conduct thorough patient assessments and medical histories.

+ Perform diagnostic evaluations and develop individualized care plans.

+ Collaborate with the healthcare team to coordinate patient care and treatment.

+ Maintain accurate medical records using EPIC or similar electronic health record systems.

+ Triage patients effectively to prioritize care based on urgency.

+ Provide education to patients and families regarding health management and wellness strategies.

+ Participate in ongoing professional development and training.

Position Details

Qualifications

+ Primary Care: 1 year experience (Required)

+ Strong knowledge of internal medicine practices and procedures.

+ Experience with medical records management, including proficiency in EPIC.

+ Ability to perform essential procedures.

+ Excellent communication skills with a compassionate approach to patient care.

+ Strong critical thinking skills for effective diagnostic evaluation and triage of patients.

Education

Bachelor's Degree-Physician Assistant (Required), Master's Degree-Physician Assistant (Preferred)

Experience

Certification(s) and License(s)
