    **Schedule:** Full Time

    **Shift** : Monday - Friday 8:00am to 5:00pm

    **Location:** Hybrid, Maitland, FL

    The role you'll contribute:

    Responsible for creating and driving the strategic plans for the clinical operations of value-based programs, contract management, and risk readiness initiatives. This position leads partnerships with employed physicians and APPs, as well as independent physicians and APPs contracted with AdventHealth sponsored clinical integration networks and accountable care organizations, to implement, track, monitor, and improve risk readiness, revenue opportunities, department activity tracking, and practice and provider performance initiatives. Additionally, this role builds strategic plans for improving risk readiness at the practice and physician level through provider engagement and creating partnerships across department and service lines. In addition, this position plans and implements education, training, data reporting, revenue tracking, and panel and contract management across all systems. Responsibilities for this role include ensuring providers and practice staff are engaged and delivering quality cost-effective care. Overall, this position contributes to the strategic development and operational success of the Primary Health Division and the organization's value-based contracts.

    The value you'll bring to the team:

    + Strengthens and optimizes workflow efforts, advising on structures and building dedicated teams for risk readiness and value-based operations.

    + Oversees planning and implementation of education, training, and communication related to physician value-based performance scorecards, risk reports, satisfaction results, and other critical operational reports.

    + Partners with support areas to inform and enforce business process and team/project governance methodologies and best practices.

    + Shapes early workflow planning and refines processes throughout the project lifecycle up to hand-off to implementation teams.

    + Facilitates activities across functional teams to scope projects, identify resource requirements, articulate deliverables, and develop timetables in partnership with Project Managers and Practice Transformation.

    + Supervises communications, contracts, and panel management teams to support initiatives and improve value-based operations at the practice level.

    The expertise and experience you'll need to succeed:

    Minimum qualifications:

    + Bachelor's degree

    + Related work experience

    Preferred qualifications:

    + Master's degree
