• **What You’ll Do:**
+ Deliver high-quality physical therapy in patients’ homes
+ Assess, plan, and implement personalized care plans
+ Collaborate with interdisciplinary teams
+ Document care thoroughly and accurately
+ Travel within Braintree and nearby areas
• **What You’ll Need:**
+ APTA-accredited PT degree
+ Active MA PT license
+ 1+ year of experience in a healthcare setting
+ Reliable transportation & willingness to travel up to 50%
+ Ability to lift 50–100 lbs and work in a physically demanding environment
• **Service Area:** Braintree, Quincy, Weymouth, Milton, and surrounding towns

You will report to the Branch Director, Clinical Manager, Clinical Supervisor or designee.
