    Summary:

    This individual will work in the outpatient setting at Penn Medicine for the **Adult Congenital Heart Disease (ACHD)** program. The incumbent will provide a professional continuum of services to address the psychosocial needs of patients and families affected by ACHD.

    Responsibilities include, but are not limited to:

    + Psychosocial evaluations to determine general social work needs for ACHD patients.

    + Assistance regarding medical insurance, disability, and/or financial aid.

    + Assistance in transition clinic at the Children’s Hospital of Philadelphia to ensure seamless transfer of care for patients/families to Penn.

    + Assessment of patient/family support needs regarding transportation, housing, clothing and educational vocational rehabilitation, social service programs and referral to community resources.

    + Crisis intervention to patients/families as necessary to meet immediate psycho-social needs including: communications from on-call provider/Emergency Department/Family Notification; trauma; family intervention; unanticipated change in health conditions/suicidal ideation; single parent pregnancy/parenthood, domestic violence, child and elder abuse/neglect.

    + Grief/loss counseling and education regarding: fetal demise; actual/pending death of patient/family member; complicated pregnancies; postpartum depression; chronic/acute stress associated with life circumstances; divorce/separation; loss of independence associated with illness or disability.

    + Short-term psychosocial support regarding but not limited to issues of adoption, abortion, domestic violence, family of origin, sexual/gender issues, substance abuse, parenting, disabilities, chronic illness.

    + Provisions of patient advocacy and associated services to patient/family and other involved agencies/parties regarding decision-making; advance directives; ethics consultations; court holds; guardianship/conservatorship; citizenship cases; and social cultural factors.

    + Attend multidisciplinary team meetings to maintain a continuum of care between inpatient and outpatient care domains and collaborate with patient/family/team/extended care providers, and insurers. The Social Worker is expected to practice in accordance with performance standards, core values and with a commitment to service excellence.

    MINIMUM REQUIREMENTS:

    + MSW required; plus one-year of experience in a medical setting

    + LSW/LCSW recommended

    + Health care experience is preferred.

    + PA license required
