    My Choice Louisiana (MCL)is a state program within the Louisiana Department of Health, jointly operated by the Office of Aging and Adult Services (OAAS) and the Office of Behavioral Health (OBH). Established under an agreement with the U.S. Department of Justice, MCL helps Medicaid recipients with serious mental illness transition or remain in community settings rather than nursing facilities. Transition Coordinators work remotely across the state to assess needs, arrange supports and services, and manage each person’s move to community living.

     

    As a Program Monitor/ My Choice Transition Coordinator , you’ll play a vital role in helping individuals move from nursing facilities to independent community living. This work-from-home position offers the opportunity to collaborate with state and local partners, coordinate transition teams, and make a real difference in the lives of Louisiana residents with serious mental illness.     Minimum Qualifications

    Five years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

     

    Six years of full-time work experience in any field plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

     

    A bachelor’s degree plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

     

    An advanced degree or a Juris Doctorate plus one year of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.

    EXPERIENCE SUBSTITUTION:

    Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

    Job Duties and Other Information

    Job Duties:

    + Coordinate and Manage Transitions:

     

    Plan, implement, and monitor the transition of individuals from nursing facilities to community settings within assigned regions.

     

    + Conduct Assessments and Develop Plans:

     

    Perform standardized assessments, determine eligibility for OAAS home- and community-based services, and prepare individualized transition plans.

     

    + Facilitate Collaboration and Communication:

     

    Lead and coordinate transition team activities with OAAS staff, MMCOs, support coordination agencies, and behavioral health partners to ensure smooth transitions.

     

    + Support Housing and Service Access:

     

    Assist participants in identifying housing options, completing housing or subsidy applications, and securing needed community-based services.

     

    + Monitor Progress and Resolve Barriers:

     

    Track cases, identify and address transition barriers, conduct pre- and post-transition visits, and evaluate transition effectiveness.

     

    + Provide Education and Technical Assistance:

     

    Offer guidance and training to nursing facility staff and transition partners, and provide troubleshooting support for complex cases.

     

    + Build and Maintain Partnerships:

     

    Develop and sustain collaborative relationships with local Long-Term Care Ombudsman staff, property managers, and other professionals to support successful community transitions. Maintain an up-to-date inventory of community resources, vendors, and organizations to assist participants in accessing services and products efficiently and cost-effectively.

     

    + Contribute to Program Improvement:

     

    Identify and recommend policy updates related to nursing facility transitions and participate in cross-agency activities to enhance program effectiveness.

    The ideal candidate should possess the following:

    Knowledge of long-term care service delivery to older adults and/or people with disabilities.

     

    Experience with one or more of the following: transition planning from nursing facilities; transition planning from ICDFs/DD; service delivery to persons with mental illness; experience with support coordination/case management; social services coordination; Medicaid or program office functions on the state or regional level; HCBS enrolled provider agency management.
