Job Description

Metro is looking for a motivated and passionate **PER DIEM** Physical Therapist Assistant to provide in-home care under the supervision and direction of a licensed Physical Therapist. Responsibilities:

+ Accept and treat patients within your designated geographic area

+ Provide physical therapy treatments to patients in their homes, as outlined in the licensed PT’s plan of care

+ Monitor and report changes in patient status to the supervising PT in a timely manner

+ Communicate effectively with patients, caregivers, and interdisciplinary team members to ensure coordinated, effective care

+ Instruct patients, family members, caregivers, and Home Health Aides in exercises, safety awareness, and use of assistive devices as directed by the supervising PT

+ Assist the PT in updating care plans based on patient progress and response to treatment

+ Document patient treatments and observations accurately and promptly, in accordance with agency and regulatory standards

+ Participate in case conferences and contribute to the care planning and discharge process, in collaboration with the PT

+ Adhere to all federal, state, and local regulations as well as agency policies and professional standards of practice

Qualifications

+ Physical Therapist Assistant Program Graduate

+ Licensed Physical Therapist Assistant in the state of New York or eligible.

+ Mandatory Certifications: First Aid, AED, & CPR Certifications.

+ Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.

+ Ability to travel to patients’ homes within the assigned geographic territory
