Summary
As a Home Health / Hospice Aide, you will provide compassionate, paraprofessional care to clients in their homes. You’ll follow care plans developed by the primary nurse and prescribed by the physician, ensuring compliance with state regulations and the Nurse Practice Act. Your role is essential in supporting clients’ comfort, dignity, and well-being.
What You’ll Do
+ Deliver personal care services such as bathing, grooming, dressing, and mobility assistance.
+ Assist with household tasks including meal preparation, light housekeeping, and laundry.
+ Monitor and record vital signs as directed.
+ Support clients with self-administration of medications (as assigned by RN).
+ Help with equipment use and safety needs.
+ Participate in case conferences and maintain accurate documentation of services.
+ Communicate changes in client condition promptly to the RN supervisor.
+ Maintain professional relationships with clients and families.
+ Adhere to agency policies, compliance standards, and ethical guidelines.
Qualifications
+ High School diploma or GED.
+ Completion of a federally compliant training program and competency evaluation.
+ At least one year of experience as an aide in home care, hospice, or an institutional setting.
+ Ability to work independently with minimal supervision.
+ Current driver’s license and reliable transportation.
+ Commitment to ongoing learning and adherence to agency policies.
+ Physical ability to lift up to 35 lbs and perform frequent standing, walking, and occasional bending or positioning of patients.
