    + Salary: $27/hour

    + Schedule: 9AM-5PM, Monday- Friday

    + Location: 320 Carleton Ave suite 8000 in Central Islip

    + Reports to: Program Director

    What drives the core purpose of this job:

    The Case Manager for Guardianship is responsible for all aspects, including personal needs and/or property management of home-based intervention services and case management for incapacitated adults in the Community Guardianship Program.

    What are the key responsibilities:

    + Within 72 business hours of assignment, contact the relevant parties to begin the fact-finding process and begin developing a service plan, consisting of social, administrative, medical, fiscal, and legal services for Guardianship clients.

    + Provide ongoing case management support including home visits, telephone reassurance, advocacy and encouragement for clients struggling with problems such as substance abuse, mental illness, housing and finances.

    + Respond promptly to client crises and emergencies, ensuring appropriate interventions are secured.

    + Provide transportation and escort clients in personal vehicle if necessary to and from appointments when needed to obtain emergency resources or to complete applications.

    + Participate in regular case conferences to support collaborative review and informed decision-making.

    + Maintain accurate, up-to-date case documentation and progress notes.

    + Communicate with judges, referees, lawyers, and DSS caseworkers regarding program participants as well as presenting to community referral sources.

    + Complete and submit Initial Report, Interim Reports, Annual Reports and court updates for clients.

    + Submit billing documentation and progress notes to the Finance Department in a timely manner.

    + Develop measurement strategies to evaluate the effectiveness of the programs.

    + Perform additional duties as assigned by Program Director and EAC Network administration.

    Work Environment:

    + Regular travel throughout assigned areas to conduct visits and attend appointments.

    + May involve handling crisis situations or responding to emergencies outside standard business hours.

    + Collaborative environment involving coordination with supervisors, social workers, attorneys, healthcare providers, and external agencies.

    What key knowledge, skills, and abilities are required to excel in this role:

    + Bachelor’s Degree in social or behavioral sciences preferred.

    + Proficiency in Microsoft Office Suite and case management or database software.

    + Minimum of two years of relevant case management or human services experience preferred.

    + Strong interpersonal, communication, and advocacy skills

    + Strong critical thinking and problem-solving skills, with the ability to assess complex situations and make sound, timely decisions.

    + Strong organizational and documentation skills.

    + Ability to motivate and support vulnerable clients with compassion and professionalism.

    + Must have access to automobile and possess a valid NY State Driver’s license.
