Job Title: Physical Therapist
Schedule: 20 hours/week (Monday–Friday, 8:00 AM–4:00 PM)
Duration: 13 weeks
Summary:
The Physical Therapist is responsible for evaluating students and adults with mental disabilities, developing individualized treatment plans, and providing therapy services in accordance with physician orders and established care plans. The role includes documentation of services, collaboration with interdisciplinary teams, and promoting wellness and independence through therapeutic interventions.
Essential Duties:
+ Perform evaluations and develop individualized treatment plans.
+ Deliver therapy services and maintain accurate documentation (screenings, progress notes, IEP reports).
+ Adapt and maintain specialized equipment and assistive devices.
+ Provide training to staff and consultation to caregivers.
+ Participate in IEP meetings and team collaboration.
Minimum Requirements:
+ Current state licensure as a Physical Therapist.
+ Graduate of an accredited Physical Therapy program.
+ One year of professional experience preferred.
+ Current CPR certification (if applicable).
+ Health certificate, TB screening, and compliance with federal, state, and local requirements.
+ Must be at least 18 years old.
Preferred Qualifications:
+ Pediatric and/or mental disability experience.
+ Strong communication, documentation, and organizational skills.
Physical Requirements:
+ Ability to lift/push up to 50 lbs, stand/walk for extended periods, and perform physical interventions as needed.
