SUMMARY OF JOB
The Clinical Care Coordinator is responsible for assisting the Unit Manager in the clinical management of the operations of the assigned units. The primary responsibility is to work under the direct supervision of the unit manager in order to facilitate resident/patient care delivery and flow through the continuum of care. The Clinical Care Coordinator must be able to perform all aspects of the nursing process, and accept resident/patient assignments in accordance with the Unit needs. Clinical coordinator must be able to recognize, interpret, document and demonstrate skills as required by the particular resident/patient population. Under the direction of the Unit Manager and/or in conjunction with the Director of Nursing, assists in implementing, monitoring, and improving clinical operations in order to achieve goals of quality, service and cost-effective deliverance of care. Responsibilities also include all aspects of the Licensed practical nurse such as communicating, collecting data, monitoring, reporting, contributing to assessments, in-serving and providing care for residents/ patients. The Clinical Care Coordinator follows departmental policies/procedures and the Pennsylvania Nurse Practice Act.
RECRUITMENT REQUIREMENTS
Graduate of an accredited School of Nursing
LPN licensed in the Commonwealth of Pennsylvania
IV Certification required
BLS Certification required
At least 2 years of staff nurse experience in a long term/rehabilitative care setting
Must be able to work alternating shifts and be flexible
Demonstrates above average performance standards
