Job Highlights:
The Footprints pediatric palliative care team supports children in the NICU, PICU, oncology and general medical floors who have serious illnesses, medical complexity, technology dependence and sometimes uncertain prognoses. We support patients and their families through supportive listening, interpreting complex medical information, helping families make difficult decisions, facilitating mutual understanding and shared decision making about goals of care. We serve families across both inpatient and outpatient settings.
Some of our patients enroll into home-based hospice. The palliative nurse will serve an important role in assisting with the enrollment process, helping to navigate insurance eligibility, and coordinating outpatient needs.
As our population grows in numbers and complexity, the palliative nurse will also assist in development and management of a patient data base for tracking productivity, outcomes and other quality metrics.
Primary responsibilities:
+ Work closely with the interdisciplinary palliative care team of NPs, MDs, SW and RN.
+ Bedside visits with patients and their caregivers enrolled in palliative care, to assess families’ understanding of complex information, facilitate a shared understanding of treatment goals, provide emotional support.
+ Telephone follow-up with caregivers.
+ Communicate and collaborate with the medical teams throughout the hospital.
+ Participate in interdisciplinary team meetings and care conferences.
+ Coordinate outpatient needs for patients enrolling in home hospice.
+ Provide bereavement/grief support to caregivers in person and by phone.
+ Document all patient encounters accurately and in a timely manner.
+ Participate in nursing education.
+ Data entry, tracking and management of the pediatric palliative care patient data base.
Requirements:
+ Excellent listening, interpersonal and communication skills with parents, children, and professional colleagues.
+ Ability to work independently, as well as part of a collaborative team.
+ Proficiency in electronic health record (EHR) documentation.
+ Engagement in innovation, research, education and lifelong learning.
+ Open to feedback.
Qualifications:
+ Minimum of 3 yrs bedside nursing, pediatrics preferred but not required.
+ Hospice, palliative care, bereavement experience and/or training preferred.
Role Specifics:
+ Schedule: Full Time, Day Shift, typically 8am - 5pm
+ Sign On Bonus: $15,000 with two-year service commitment for external qualified candidates

Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
+ Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
+ Collaborates with physicians and advanced practice providers to ensure quality outcomes. Coordinates efforts for treatment and promotes quality improvement and educational efforts.
+ Compiles data, tracks outcomes, and makes recommendations for process improvement.
+ Provides referrals/support for follow-up care for patients who are uninsured or underinsured.
+ Utilizes the nursing process of assessment, planning, intervention, and evaluation in all patient encounters.
+ Assists medical provider with processing of diagnostic results bringing significant values to the provider’s attention. Exhibits knowledge of tests and procedures and their significance as related to patient care.
+ Recognizes the implementation of dosages, interactions, side effects, adverse effects, routes of administration of drugs as they apply to individual patients. Uses knowledge of medications in instructing patients about medication therapy.
+ Establishes working relationships teaching protocols for patient education, family counseling and general public information.
+ Works in a constant state of alertness and safe manner.
+ Performs other duties as assigned.
EDUCATION
+ Graduate of accredited school of nursing or education equivalency for licensing
EXPERIENCE
+ Three years' registered nurse experience
PHYSICAL REQUIREMENTS
+ Constant use of speech to share information through oral communication.
+ Constant standing and walking.
+ Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
+ Frequent sitting, reaching and keyboard use/data entry.
+ Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
+ Frequent use of smell to detect/recognize odors.
+ Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
+ Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
+ Occasional lifting/moving of patients.
+ Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
+ Occasional driving.
+ Rare crawling and running.
