Job Summary:
Reentry Specialists (RS) – The RS, under the direct supervision of the Office of Reentry Services Campus Program Coordinator, will monitor transition services in facilities of assignment. The RS will review the youth’s progress in the service plan goals and collects other pertinent information for decision making regarding the youth’s release from secure confinement. Other duties will include but not be limited to the following:
Primary Duties & Responsibilities:
+ Manage an assigned caseload of youth at Augusta YDC and/or youth screened for YDC held at RYDC and are 120 days from release in accordance with policy 25.1
+ Initiate the Domains of Aftercare (DOA) module in JTSnx within three days of youth’s initial YCRT meeting and will update the DOA as needs change
+ Review and update the Reentry checklist in JTSnx within three days of each YCRT meeting (ensuring that all Team members complete their tasks to transition youth)
+ Ensure that the YCRT members are being proactive in putting forth eligible youth for early release consideration and step-down eligibility
+ Providing oversight and support to youth obtaining a state id if needed
+ Reviewing service plans and preparing for the transition with youth's case manager and Reentry Resource Coordinator (especially around career/resource planning/ treatment needs/workforce readiness programs/Social Security card and benefits/housing)
+ Coordinating with Residential Placement Specialists on available resources for youth being released to residential placement
+ Facilitating all YCRT -Youth Centered Reentry Team Meetings
+ Provide training associated with Reentry and Transitional Services for DJJ Staff and YCRT members.
+ Monitoring progress of Youth Reentry Checklist and service planning goals.
+ Responsible for performing monthly Quality Assurance reviews for all youth releasing from the assigned facility and submitting formal reports.
+ Facilitate connectivity to community resources when necessary to support a seamless transition.
+ Ensure that there is a focus on families for youth being released insuring that family supports are available upon release from custody.
+ Utilize a statewide database to connect youth to services when there is a need.
+ Ensure that transition activities support the five dimensions of overarching case management including 1) Assessment and Classification, 2) Individual Case Planning, 3) Surveillance/Service Mix, 4) Incentives/Consequences (Graduated Responses), and 5) Brokerage/Linkages.
+ Through the use of an ORS approved Quality Assurance Tool, ensure that aftercare plans address the following domains :
• Family and living arrangements
• Peer groups and friends
• Mental, behavioral, and physical health
• Substance abuse
• Education and schooling
• Vocational training and employment
• Recreation and vocational interests
Minimum Qualifications:
+ Bachelor’s degree in a related field from an accredited college or university and two (2) years of job-related experience.
+ Or two (2) years of experience at the lower-level Social Services Program Coord I (SSP030) or position equivalent.
Preferred Qualifications:
+ Master’s degree in education, counseling, social work, criminal justice or related field from an accredited college or university
+ Minimum two years youth/adolescent case management experience; Knowledge or prior experience working with justice involved youth or adults

• Statewide travel as required
• Moderate lifting as required, up to 30 pounds.
