• Social Work Associate

    Charles B. Wang Community Health Center (Queens, NY)


    + Work with clinical social worker to provide social work services at the service units

    + Complete psychosocial assessment for patients as indicated

    + Formulate and implement the intervention plan for patients

    + Provide concrete case management, follow-up and referral service

    + Provide entitlements/benefits eligibility counseling and assist with application

    + Assist Clinical Social Workers in providing case management service as needed

    + Timely services provided to patients based on the Social Work department guideline

    + Complete documentations and reports based on the Social Work department guideline

    + Attend case conference, agency meetings and staff meetings

    + Participate in community outreach and marketing activities

    + Perform other duties as assigned
