Description:
SUMMARY
Responsible for providing clinical pharmacy services in an outpatient and/or inpatient practice area, by providing drug therapy
management and assessment for patients in the practice area. Provides support for established programs including, but not limited to,
drug formulary, pharmacy education programs, and quality improvement and research relating to drug therapy.
QUALIFICATIONS
Education
Minimum Doctor of Pharmacy
Credentials/Licensure
Minimum Pharmacist license and controlled substance license issued by the Michigan Board of Pharmacy. Basic
Cardiac Life Support (BCLS) certification may be required (at the discretion of manager).
Related Experience
Minimum Completion of the requirements for advancement through the Professional Advancement Program to
Level IV
Preferred Clinical pharmacy residency plus one to two years related experience
Other Knowledge, Skills and Abilities
Minimum
Preferred Flexible/on-call scheduling may be required.
Computer Competency:
Familiarity with standard desktop and windows-based computer system, including email, e-learning, intranet and computer
navigation. Ability to use other software required to perform essential functions.
Physical/Mental
Minimum Ability to use a computer keyboard. Sitting, walking, or standing for long periods of time. Ability to work
with frequent interruptions and meet aggressive deadlines. Ability to work independently and
cooperatively with health-system and pharmacy staff.
WORKING CONDITIONS
Potential for exposure to infectious patients and materials. Potential for exposure to hazardous and toxic substances (including
chemotherapy, cytotoxic drugs, and cleaning solutions). Potential for sticks or cuts by needles and other sharp items.
ESSENTIAL FUNCTIONS
• Provides drug therapy management and assessment.
• Identifies and resolves potential and actual drug related problems.
• Assists physicians in designing dosing regimens based on patient-specific dosing parameters.
• Communicates and collaborates with physicians and other clinicians to develop drug therapy policies, protocols, and
guidelines.
• Provides support to the Pharmacy & Therapeutics Committee which may include serving as a member and/or
activities such as preparing formulary drug reviews, medication usage evaluations, policies and guidelines for drug
therapy, etc.
• Participates in performance improvement initiatives and research as appropriate.
• Participates in departmental, multidisciplinary, hospital and health-system committees as leader, member or
consultant.
• Provides education and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians,
nurses, students and other clinicians.
• Participate in the training, education, precepting, and evaluation of pharmacy students and/or residents as assigned.
• Performs other related duties as assigned such as quality assurance reports, preparation for regulatory inspections, etc.

To ensure appropriate utilization of Protected Health Information (PHI) associated with the Health Insurance Portability and
Accountability Act (HIPAA) and Saint Mary’s Confidentiality of Information Standards, as well as other regulatory entities,
individuals employed in this position will be granted systems and information access as appropriate for this position.
