Jennie Stuart Health is seeking a dedicated and compassionate BHU Social Worker to join our Behavioral Health Unit team as a Primary Therapist. This integral role involves providing high-quality psychotherapy and comprehensive social services to clients and their families, with particular emphasis on treatment implementation and discharge planning. The successful candidate will be responsible for maintaining current knowledge of all relevant regulations and standards governing social services.
As a BHU Social Worker at Jennie Stuart Health, you will serve as the primary point of contact for discharge planning, working collaboratively with the Treatment Team to ensure seamless transitions for clients. Your responsibilities will include conducting thorough interviews with clients and their families to obtain comprehensive social histories and pertinent data for treatment and discharge planning. You will interpret various social situations as they relate to clients' conditions, admission circumstances, and post-discharge needs, modifying social services plans accordingly to ensure optimal outcomes.
The role requires active participation in Treatment Team meetings and the ability to conduct both individual and group therapy sessions as directed by the Medical Director, physicians, or Treatment Team. You will address issues identified in social histories that may affect treatment progress and discharge planning. Additionally, you will facilitate support groups as needed and represent the program at educational meetings when requested by the Program Manager.
As the discharge planning specialist, you will be responsible for accurately completing all forms and documents related to client discharges, collaborating primarily with RNs and Unit Clerks to ensure comprehensive documentation. Working cooperatively with the Unit Manager, Unit Director, and Clinical Director, you will ensure compliance with all relevant regulations and standards, including HFAP, Medicare, and state licensure requirements.
Data collection and reporting are significant aspects of this position. You will assist in monitoring and evaluating psychotherapy services, social services, and discharge planning processes as specified in the Quality Improvement Plan. Additionally, you will collect ongoing cognitive and affective testing data as requested by the Medical Director.
This is a full-time, regular position located at the JSH Medical Center. The position is classified as regular/at-will employment. The ideal candidate will possess extensive knowledge of community resources for older persons, Medicare, Medicaid, and insurance requirements as they relate to continuing care. Familiarity with medical terminology, conditions affecting treatment and discharge needs, medical team roles, and the functioning of other hospital departments is essential. Strong communication skills and knowledge of effective individual and group communication techniques are crucial for success in this role.

Required Skills

Required Experience
+ Master's degree in Social Work from an accredited Graduate School of Social Work

+ Experience in behavioral health settings

+ Experience with treatment planning and implementation
