Proposed Schedule: 40 hours per week. Monday-Friday 8am-4:30pm.
Note: This positions home location will be Plattsburgh Family Health but will also be expected to travel across our northern region of Health Centers as necessary.
Position Summary
The Clinical Pharmacist will be responsible for supporting the Network’s clinical pharmacy services to help improve patient outcomes and ensure safe, cost-effective drug therapy. The position will support a coordinated and high-quality medication therapy management program through the delivery of comprehensive medication reviews and patient-centered pharmaceutical care.
Essential Duties and Responsibilities include, but are not limited to:
+ Provide medication therapy management (MTM) services for high risk patients including comprehensive medication reviews, medication adherence counseling and other targeted interventions
+ Reconcile medications for identified high risk patients undergoing transitions in care
+ Collaborate as an extended care team member within the health center to assist patients and clinicians in meeting medication-related treatment goals
+ Participate in pharmacy incentive programs with payors and collaborate with payors on value-based care programs, as appropriate
+ Document care provided in patients’ records and update medication lists as appropriate
+ Provide medication-related information to patients and clinicians and offer education, where appropriate
+ Additional duties may be assigned to meet business needs

+ Ability to communicate effectively with patients and colleagues from varying levels of medical backgrounds
+ Possess strong clinical, interpersonal, and organizational skills with a strong attention to detail
+ Proficiency in the use of Microsoft Office Suite, especially Excel, Outlook, PowerPoint and Word
+ Proficiency in the use of OutcomesMTM® Connect™ platform
+ Familiarity with electronic medical records and the ambulatory care environment
Qualifications:
The requirements listed below are representative of the knowledge, skill and ability to perform the essential functions:
+ Valid Pharmacist license and current registration with eligibility for licensure in New York State
+ Doctor of Pharmacy Degree with at least two years of clinical experience preferred
+ Immunization Certification desirable
+ Current BLS certification
+ Board certification preferred (or willing to obtain within 2 years of hire)
The salary for this position is $130,000/year
