Help us elevate patient care to a whole new level! Adecco is assisting our client to locate a **Case Management Coordinator** to join their team—an industry leader in serving dual-eligible populations using best-in-class clinical and operational models.
In this role, you’ll directly support members enrolled in Medicare and Medicaid who face a wide range of complex health and social challenges. Through compassion, active listening, and strong communication skills, you’ll partner with members, providers, and community organizations to address the full continuum of health and social determinant needs.

What You’ll Do
+ Use critical thinking to support and advance the case management process
+ Help coordinate member care by scheduling appointments
+ Identify available benefits and assist members in accessing them
+ Provide education and resources using care management tools
+ Collaborate closely with internal teams, providers, and community partners
+ Advocate for members with empathy and professionalism
What We’re Looking For
Bachelor’s degree in a behavioral health or human services field (REQUIRED)

**Pay Details:** $26.00 to $28.57 per hour
