Supervisory LPN will support home care clients and caregivers throughout our Atlanta, GA territory.

Job Summary:

Under the guidance of the Clinical Leader and local market leadership, the Supervisory LPN collaborates with local and market teams to:

+ Perform in-home assessments to determine support and client needs

+ Conduct regularly scheduled supervisory visits of clients

+ Monitor personal care and companion care services provided by direct care staff

+ Assist in educating direct care staff to ensure quality of care

+ Maintain confidentiality regarding client information and all related documentation

Qualifications:

+ State of Georgia or multistate LPN licensure

+ Current CPR certification

+ One (1) year of clinical nursing experience; home care experience is a plus

+ Excellent interpersonal, written, and verbal communication skills

+ Energetic team player with a positive attitude
