    *_SUMMARY_*

     

    Hennepin Healthcare is seeking a*full-time Staff Nurse (RN)*to join our*Physical Medicine & Rehabilitation (PMTR) and Traumatic Brain Injury (TBI) programs*. This daytime position (Monday–Friday, 8:00am–4:30pm) plays a vital role in supporting the*TBI Outpatient Program*and the*PM&R clinic*. The Staff Nurse will focus on*care coordination*, assisting patients as they transition from hospital or Knapp Rehabilitation to outpatient care, ensuring continuity of services and comprehensive support throughout recovery. In addition, the nurse will provide*clinical support for injections and procedures as needed*, contributing to safe and effective patient care.

     

    *_RESPONSIBILITIES_*

    Within the context of the Hennepin Healthcare System Relationship-Based Care practice model, and under general supervision, the registered nurse provides and coordinates nursing care by:

    * Protecting, promoting wellness and optimizing health

    * Triaging patients by assessing severity of symptoms and referral to appropriate setting or level of care

    * Alleviating suffering through the treatment of the human response

    * Partnering with the patient, their family and significant others

    * Documenting interventions and an individualized plan of care

    * Educating patient on their disease process

    * Assuring continuity of care

    * Educating patient and family in preventative care which promotes health and well being

    *_QUALIFICATIONS_*

    * Graduation from an accredited school of nursing

    * Possession of a valid license as a Registered Nurse issued by the State of Minnesota

    * Certification in area of clinical specialty

    * BSN Preferred. Bachelor of Nursing degree is strongly encouraged. Advancement in academic education will be discussed as part of individual professional development during the annual performance review.
