    Responsibilities

    Provide social services to patients, families and the community as requested or deemed appropriate.

     

    + Understand and implement social work principles and code of ethics.

    + Conduct discharge planning, crisis counseling, community resource information and education.

    + Assess the patients' services to determine whether they are successful.

    + Work effectively within a multidisciplinary team environment.

    Requirements

    Licensure / Certification / Registration:

    + Social Worker credentialed from the Texas Department of State Health Services required.

    Education:

    + High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree with 1 year of experience OR 5 years of applicable cumulative job specific experience required.

    Additional Preferences

    + LCSW preferred.

    + Experience as a clinician working in a hospital on a interdisciplinary team preferrred.
