Position Summary
The Team Leader is responsible for overseeing a multidisciplinary ACT clinical team that provides treatment, rehabilitation, and support services to individuals with severe and persistent mental illness. This role includes program oversight, staff supervision, clinical leadership, quality assurance, and direct client care.
Essential Responsibilities
Team Leadership & Supervision
+ Lead daily team meetings; provide clinical direction and case guidance.
+ Conduct regular individual and group supervision.
+ Monitor staff productivity and provide coaching to ensure targets are met.
+ Participate in recruitment, interviewing, hiring, onboarding, and performance evaluation.
+ Provide disciplinary actions in accordance with agency and program policies.
+ Model professional behavior and foster a collaborative team culture.
Program Operations & Compliance
+ Oversee client admissions, treatment planning, and delivery of ACT services.
+ Ensure timely and accurate documentation in the EHR (Avatar) and VelloHealth.
+ Maintain program policies and procedures; update as required.
+ Ensure compliance with agency, OMH, and AOT regulations.
+ Conduct regular chart audits to ensure quality and compliance.
+ Participate in required OMH ACT orientations and trainings.
Client Care & Community Coordination
+ Provide direct clinical services, including crisis intervention and counseling.
+ Maintain 24-hour on-call availability for emergencies and staff consultation.
+ Conduct field-based visits; ACT services occur primarily in the community (approx. 85%).
+ Coordinate services with AOT, mental health/criminal courts, probation, child protection, and other external partners.
+ Act as a clinical liaison with outside agencies.
+ Participate in case conferences and ensure high-quality service delivery.
+ Complete all progress notes within 48 hours of client or collateral contact.
+ Conduct 30-, 60-, and 90-day follow-ups for discharged clients.
Client Skill-Building & Support
+ Assist clients in developing independent living skills, including budgeting, grooming, nutrition, transportation use, community integration, shopping, housekeeping, physical health, vocational goals, and social development.
+ Escort clients to appointments as needed (medical, psychiatric, substance use).
+ Support clients in identifying and accessing community resources.
Other Responsibilities
+ Maintain accurate documentation and communicate effectively with staff and supervisors.
+ Work a flexible schedule based on program needs.
+ Operate an agency vehicle as required.
+ Attend mandatory trainings and meetings across various sites.
+ Provide coverage during staff shortages.
+ Maintain strict confidentiality.
+ Report and document all incidents to the supervisor within 24 hours.
+ Perform additional duties as assigned by leadership.

Qualifications
• Master degree or higher in social work, psychology, rehabilitation counseling, or a related field, with licensure/registration. LMSW, LCSW, LMHC, RN (RNs are encouraged to be masters level or nurse practitioner), or physician (Ph.D, Psy. D, M.D.). Preference will be given to persons with experience on a multidisciplinary mobile team.
• Two years supervisory experience with all levels of staff. Experienced in meeting regulatory requirements with OMH and other governmental agencies and working with behavioral health population preferred.
• Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record.
• Strong interpersonal and communication skills and knowledge of group dynamics required
• Valid New York State driver’s license and vehicle required – candidates are subject to regulatory background checks.
