Overview
Purpose: As an advocate for the patient, the Clinical Patient Advocate assists patients and their family members with their needs
The Clinical Patient Advocate must possess strong communication and interpersonal skills. This advocate supports and assists the Director of Patient Experience in coordinating empathetic and timely relations between patients and their families throughout all areas of the Emergency Department. Supports and promotes patients' rights as they navigate the health care system.
The Clinical Patient Advocate collaborates well with other disciplines and is responsible for enhancing the process of communication between the patient/family and the healthcare staff throughout the Emergency Department. Shows good judgment in setting priorities to determine the most important and useful areas to focus his or her efforts while consistently striving to provide exceptional patient experiences.
Job Details
The Clinical Patient Advocate demonstrates knowledge of nursing practice and uses this knowledge to assist the patient/family and healthcare team to achieve common goals and helps resolve issues and concerns when they occur. Responsibilities include; but are not limited to:
+ Greet patients and family members upon arrival
+ Provide comfort care and communication for patients
+ Keep patients informed of delays
+ Conduct frequent rounds to determine actual and potential needs of patients and their family members
+ Identifying and attending to any special patient needs
+ Showing concern and maintaining patients privacy
+ Function as a liaison between patients, families, physicians, clinicians and staff
+ Maintain open communication and effective working relationship with other members of the hospital health care team.
The Clinical Patient Advocate will provide support and care to the patient and family members as directed by the Nursing Supervisor and/or Patient Experience Department. The advocate will be responsible for communicating to the patient and family members the plan of care, collaborating with the Nursing Supervisor to ascertain appropriate bed assignment, assist in scheduling tests, doctors appointments and any other needs that may present itself to provide an exceptional patient experience for the patient and family members. The advocate will also assist the patient and/or family member as directed with scheduling tests or appointments if they should call from home.
