    Job Brief

     

    The Incident Management Investigator assists with completing thorough investigations into situations which may violate a person’s rights and/or protections in accordance with OPWDD and Justice Center regulations.

    POSITION SUMMARY:

    The Incident Management Investigator works closely with the Incident Management Investigator Specialist I (IMIS-I) and Incident Management Investigator Specialist II (IMIS-II) to conduct thorough investigations into situations which may violate a person’s rights and/or protections in accordance with OPWDD and Justice Center regulations.

    POSITION DUTIES:

    1. Works closely with the Incident Management Investigator Specialist I and II.

    2. Participates in weekly group and individual supervision as scheduled.

    3. Initiates the investigative process by developing documentation “1st Request” for distribution to program teams.

    4. Identifies potential witnesses and subjects of investigations for interview based on documentation review.

    5. Conducts investigative interviews that address all aspects of the incident reported, as well as explores any systemic concerns.

    6. Alerts the Sr. Director of Investigations of any new incidents discovered during the investigative process and reports as needed and/or as directed by the Sr. Director of Investigations.

    7. Reviews investigative files for thoroughness and coordinates follow-up requests with IMIS team members.

    8. Oversees the completion of OPWDD 149 Final Reports by making any necessary revisions to drafts completed by the IMIS team and developing conclusions and recommendations in accordance with agency and regulatory standards addressing any and all issues identified through the investigation.

    9. Completes any revisions following Incident Review Committee (IRC) Sub-Committee, OPWDD, and Justice Center (JC) reviews.

    10. Testifies as needed for any grievance or JC Appeal hearings.

    11. Escalate any obstacles to completing investigations to the Sr. Director of Investigations.

    12. Other duties as assigned.

    REQUIREMENTS:

    1. Minimum BA/BS in related Human Services Field or licensure in medically related field.

    2. Up to date certificate in approved Justice Center Investigative Training Courses.

    3. At least one year working within OPWDD or comparable human services programs.

    4. Thorough knowledge of state regulations governing incident reporting (14 NYCCR Part 624 and 625, Article 20 of the Executive Law and Article 11 of the Social Services Law), Protection of Individual in Programs certified by OPWDD (Part 633 regulations), and the Justice Center PPSNA (Protection of People with Special Needs Act).

    5. Typing and computer proficiency, specifically experience working with Microsoft Office Suite, One Drive, SharePoint, and Microsoft Teams.

    6. Able to review, process, and analyze evidence.

    7. Demonstrates proficiency in multitasking, effectively prioritizing tasks, consistently meeting deadlines, and readily adapting to evolving circumstances.

    8. The position requires availability from 9am to 5pm, flexibility, and timely responses to all communications.

    9. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
