Position Overview:
Assesses, diagnoses, plans and implements the medical care of patients in collaboration with a physician. Functions within the full scope of licensure; accountable for the assessment, coordination delivery and evaluation of nursing care, including direct patient care, patient/family education and transitions of care. Delivers holistic and individualized care to all patients in assigned area. Develops, implements, manages an optimal interdisciplinary plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications. Adheres to hospital polices, industry standards, best practices, and applicable laws/regulations and codes to promote a quality, highly reliable patient experience. Engages in continuous growth and development in professional practice. Follows ambulatory care and inpatient standardized procedures, protocols, policy and procedures regulated by the State of California.

JOB ACCOUNTABILITIES
Provides pharmacotherapy and supportive psychosocial therapies;
• Completes assessments and formulates treatment plans in a timely manner;
• Monitors progress towards goals.
• Evaluates patients and, as needed, provides re-evaluation.
• Works within ethical guidelines and within the clinician’s scope of practice.
• Maintains a minimum work schedule of 40 hours/week.
Develops treatment plan and, as appropriate, acts as a liaison between the patient and their internal and external care team members.
• Makes referrals, as needed, to internal and external services.
• Communicates directly with internal and external providers to ensure continuity of care.
• Provides telephone consultation to off-site providers.
• Participates in case discussions at department meetings.
Demonstrates the advanced knowledge, assessment skills, and therapeutic abilities needed to move patients from initial presentation to clinical stability.
• As needed, refers patients to the supervising physician or to another specialist.
Participates in integrated behavioral healthcare program planning, implementation, and evaluation. Collects and reports data as needed.
• Helps to maintain a current list of community resources for referral purposes.
• Performs all miscellaneous duties and participates in organization projects as assigned.
Transmits drug orders, furnishes, dispenses, and administers medications consistent with all regulations and approved policies and procedures; documents medication orders and administration according to clinic standards.
Shares on-call responsibilities with department members, including providing triage, consultation, and intervention services in other PAMF clinics.
Provides timely, comprehensive documentation of the patient assessment, treatment, problem areas, goals, and progress in the Electronic Medical Record (EHR).
• Maintains the patient’s health record in a manner consistent with the department’s quality guidelines and legal and ethical standards.
Provide patient care for virtual and face-to-face primary care
Perform high-quality primary clinical care in virtual/video visits, patient-home visits, or in-person clinic visit. May be acute, chronic, or wellness focused.
• Visit patients in acute/subacute health-care settings, as needed.
• Assist with patient health education.
• Provide occasional back-up call (including evenings and weekend) for the clinic population to support the solo clinic physician.
Contribute to care innovation
• Engage with the technology partnership engineers, design and innovation team at Sutter, and leadership team at PAMF/Sutter.
• Work closely with the lead physician to develop other projects of interest for the clinic model, where you’ll take initiative, leadership, and be accountable for outcomes.
• Assist in developing metrics to evaluate the outcomes and effectiveness of the clinic’s process improvement/innovations.
Provide clinical support for care-team
• Efficiently management Epic in-basket messages and tasks.
• Assist with pre-visit planning and preparation (pre-visit labs/tests, care gaps, and refills) for efficient face-to-face.
• Assist patients to troubleshoot straightforward technical issues with technology tools.
Assist with clinic’s administrative duties
• Assist in weekly team meetings for process improvement/quality improvement.
• Train other APCs and staff during model expansion.
• Miscellaneous duties supporting clinical operations and patient care.
Support the LVN and Health Coach
• Assist LVN and health coach with their tasks and problem solving.
EDUCATION
Master's: Masters Degree in Nursing or equivalent education/experience
CERTIFICATION & LICENSURE
NP-Nurse Practitioner
NFP-Nurse Practitioner Furnishing
CSRC- Controlled Substance Registered Certification (DEA)
ANCC - American Nurses Credentialing Center or AANP American Academy of Nurse Practitioner
BLS-Basic Life Support Healthcare Provider
PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED:
The ideal candidate will have experience working with children, adolescents, families, and adults of all ages—as well as in working within primary care settings and in using an integrated care approach. Fluency in Spanish is strongly preferred. Experience using Epic is also preferred.

Pay Range is $91.66 to $128.32 / hour
_The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package._
