    Position Summary

    The Assistant Team Leader provides direct clinical services and supports the overall supervision and administrative oversight of the ACT team. This role assists in guiding specialty staff using evidence-based practices, trauma-informed care, cultural humility, and person-centered approaches. Responsibilities include conducting individual and group supervision, supporting team operations, and collaborating closely with the Team Leader and prescriber.

    Essential Accountabilities

    + Co-lead daily team planning meetings and provide clinical direction on individual cases.

    + Provide individual and group supervision to staff and support ongoing skill development.

    + Meet productivity expectations and ensure timely completion of all required documentation.

    + Support and oversee the client admission process, treatment planning, and delivery of rehabilitation and support services.

    + Participate in staff recruitment, hiring, onboarding, supervision, and performance management.

    + Develop, implement, and revise program policies, procedures, and staff training.

    + Participate in on-call rotation for crisis intervention, staff consultation, and supervision.

    + Monitor accurate use of the electronic health record (Avatar) and ensure compliance with agency and OMH documentation standards.

    + Provide guidance to staff in consumer engagement, communication, and removal of barriers to care.

    + Conduct regular chart audits to ensure quality and compliance.

    + Coordinate services and communication with AOT, mental health and criminal courts, probation, and child protective services.

    + Administer corrective action when staff violate agency, contractual, or program policies.

    + Serve as a clinical liaison between the ACT team and external partners.

    + Document progress notes in the EHR within 48 hours of client and collateral contact.

    + Communicate key operational updates to leadership, including Deputy Director, Division Director, and regulatory entities.

    + Organize and participate in case conferences and case consultations.

    + Conduct field-based visits; ACT services occur primarily in the community (85%).

    + Participate in required OMH ACT orientations and trainings.

    + Enter and monitor data within the VelloHealth platform and ensure staff accuracy.

    + Support clients in developing independent living skills, including:

    + Budgeting, grooming, nutrition, transportation, and community resource use

    + Shopping, housekeeping, physical health, adaptive behavior, and independent living

    + Vocational and social skill development

    + Serve as a positive role model; assist clients in accessing resources and attend appointments as needed.

    + Provide crisis intervention and mental health counseling when appropriate.

    + Attend client meetings and complete 30-, 60-, and 90-day follow-ups on discharged clients.

    + Attend required meetings and trainings at various locations.

    Other Responsibilities

    + Maintain accurate documentation and communicate effectively with staff and supervisors.

    + Work a flexible schedule based on program needs; travel to multiple sites as required.

    + Operate an agency vehicle and complete all required trainings.

    + Provide coverage during staff absences or shortages.

    + Maintain confidentiality and adhere to all agency and regulatory standards.

    + Report and document all incidents to the immediate supervisor within 24 hours.

    + Perform additional duties as assigned by the Team Leader, Deputy Director, Director, or Senior Management.

    Qualifications

    + Master degree or higher in social work, psychology, rehabilitation counseling, or a related field, with licensure/registration LMSW, LCSW, LMHC, OR RN (RNs are encouraged to be masters level or nurse practitioner), or physician (Ph.D, Psy. D, M.D.). Preference will be given to persons with experience on a multidisciplinary mobile team.

    + Two years supervisory experience with all levels of staff. Experienced in meeting regulatory requirements with OMH and other governmental agencies and working with behavioral health population preferred.

    + Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record.

    + Strong interpersonal and communication skills and knowledge of group dynamics required

    + Valid New York State driver’s license and vehicle required – candidates are subject to regulatory background checks.
