Note: Licensing Requirement Must have at least 4 active licenses from the following states: NC, SC, GA, VA, PA, NY, NJ

Key Responsibilities

1. Patient-Centred Quality & Safety

+ Evaluate primary care, acute, chronically ill, and transitional care patients (18 months+).

+ Provide counselling on pregnancy prevention, STI/safer sex practices, contraceptive care, and medication management.

+ Educate patients on health maintenance and respond to patient inquiries.

+ Document all care in the EHR in accordance with Healthcare policies and procedures.

+ Coordinate care with internal and external colleagues, including the broader patient-centred medical home.

+ Collaborate effectively with fellow providers, collaborative physicians, pharmacists, and other team members **.**

2. Customer Service Excellence

+ Deliver a warm, patient-focused telehealth experience.

+ Incorporate feedback from patients, leadership, and colleagues to improve engagement and satisfaction.

+ Adapt communication style to patient needs to support quality outcomes.

+ Collaborate with pharmacy and front-store colleagues to support a seamless patient experience.

+ Maintain strict patient confidentiality in accordance with PHI and HIPAA.

3. Healthcare Environment Management

+ Use appropriate techniques to resolve conflict and support a positive work environment.

+ Build and maintain strong relationships with practice staff and internal partners.

+ Reprioritize tasks throughout the day to meet patient and business needs.

+ Balance clinical and non-clinical responsibilities to support the overall patient experience.

+ Quickly adopt new service offerings and patient care models.

+ Assist with hiring, development, and evaluation of practice employees as needed.

+ Support clinic operations (inventory, lab follow-up, phone calls, supply ordering, and maintaining a clean/organized environment).

4. Business Acumen

+ Support clinic business goals, including budget, payroll, inventory, billing, and payment collection.

+ Execute the clinic-level business plan to achieve identified objectives.

+ Ensure accurate documentation to support complete and accurate payment for services.

+ Verify payment method (insurance, cash, or combination) for each visit and collect payment per EMR instructions.

+ Perform revenue cycle tasks, including payment collection at time of service and preparing nightly deposits.

5. Autonomy & Professional Development

+ Work independently, prioritize and solve problems, and advocate for patients and practice needs.

+ Engage in ongoing professional development through committees, professional groups, and other experiences.

+ Maintain self-awareness, professionalism, and accountability for individual impact on the clinic and organization.

Required Experience & Qualifications

+ Nurse Practitioner Questionnaire must be included with the resume.

+ Minimum 2 years of medically relevant experience as an NP or in a related clinical role.

+ Current Basic Life Support (BLS) certification.

+ Reliable high-speed broadband internet with minimum 25 Mbps download / 3 Mbps upload (candidate must acknowledge this requirement).

+ Unrestricted NP license with ability to obtain multi-state / compact privileges and additional non-compact state licenses as required.

+ Strong verbal, written, and electronic communication skills.

+ Excellent organizational skills and ability to multi-task.

+ Demonstrated initiative, problem-solving, adaptability, and flexibility.

+ Ability to work autonomously without direct on-site supervision.

+ Proficiency with technology and information systems (EHR, telehealth platforms, etc.).

+ Ability to collaborate with other professionals to deliver high-quality care.

Education

+ Completion of a master’s degree level Family Nurse Practitioner (FNP) program.

+ Current National Board Certification as a Family Nurse Practitioner.

+ Active State of Employment NP license to practice in the Advanced Practice Nurse role.
