Registered Nurse - Care Transitions Clinical Supervisor

Job Description
The Care Transitions Clinical Supervisor provides essential support and leadership to the Recuperative Care nursing and support staff. This role focuses on staff development and oversight, fostering a strengths-based, client-centered culture. The Clinical Supervisor coordinates the administration of medical services to clients, including assessing, screening, educating, and treating patient conditions. The role includes overseeing medication management workflows to minimize risk and ensuring compliance with policies and best practices.
Responsibilities
+ Work as part of a multidisciplinary team with patients experiencing homelessness and co-occurring disorders, such as mental health issues, chronic medical conditions, and substance use disorders, in a high acuity care facility requiring 24/7 support and monitoring.
+ Train nursing, support staff, and new hires in nursing and patient support duties, medication management, policies, and processes.
+ Oversee nursing and support staff functions, ensuring compliance with established guidelines, protocols, and best practices.
+ Perform RN duties for patients receiving medical respite care, including preparing rooms, rooming patients, taking medical histories, recording vital signs, completing assessments, and performing venipuncture and immunizations as needed.
+ Provide education and oversight to ensure adherence to certifying and regulatory standards by all nursing and support staff.
+ Oversees training, monitoring, auditing and compliance of medication management; medication intake, logging, entry into electronic health record, documentation of medication changes, narcotic workflows, medication disposals and discharges, and patient medication calls.
+ Works directly with Clinical Risk Manager to identify errors, do root cause analysis, and develop error prevention workflows to reduce risk.
+ Trains, coaches and evaluates staff is medication management performance, errors and prevention.
+ Remains actively involved in recruiting, interviewing and hiring all nursing and support staff.
+ Provides education and best practice updates at all staff meeting and during in-services.
+ Assesses and develops strategies to address knowledge and skill gaps of nursing and support staff.
+ Provides adequate orientation, professional opportunities and study time to remediate gaps in necessary knowledge skill by coordinating with nursing supervisor.
+ Writes Recuperative Care policies and processes using best practice standards.
Essential Skills
+ CA RN License
+ 2+ years of supervisory experience
+ Proficiency in medical terminology and patient care
+ Experience with electronic medical records (EMR) systems such as NexGen
+ • Current license as an RN in the state of California, graduate of an accredited school of nursing.
+ Must have current BLS Certification or willingness to obtain within 90 days from date of hire.
+ Two plus year’s related experience in clinical experience with diverse adult population preferred; OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
+ Leadership experience, ability to supervise and mentor a team of Licensed Vocation Nurses and support staff.
+ Commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
+ Strong physical assessment skills.

+ Experience working with unhoused community members and people in a substance use, behavioral and mental health disorder.
+ Knowledge of medications, usages, interactions and effects including interactions while intoxicated.
+ Ability to observe, assess, and record symptoms, reactions, and progress; react calmly and effectively in emergency situations.
+ Educate staff, patients and/or families and to provide instruction on proper care and treatment in addition to best practices and adherence to certifying and regulatory standards of care.
+ Maintain detailed records.
+ Ability to establish and maintain positive, professional relationships.
+ Computer literacy including use of email, Microsoft Word and Excel and Electronic Health Records, preferably experience with EPIC.
+ Ability to work holidays, weekends, and alternate shifts as needed to maintain required staffing levels.
+ Participation in an on-call rotation, including responding to urgent matters outside of regular business hours. Employees must be reachable, able to respond promptly, and take appropriate action as needed. • Communicate clearly and concisely, both verbally and in writing.
+ Bilingual in Spanish or other languages desirable. • Ability to work holidays, weekends, and alternate shifts as needed to maintain required staffing levels
Work Environment
The role supervises up to 15 individuals and requires at least 2 years of experience in a supervisory capacity. The work schedule is flexible, typically Monday to Friday from 11am to 7pm, with occasional early starts around 6:00am to support night shifts. Pay and Benefits
The pay range for this position is $130000.00 - $145000.00/yr.
