    Position: RN - Case Manager

     

    Facility Type: Hospital

    Duration: 13 weeks

     

    Hours: 40

    Shifts:

    + Days, 5x8s Rot. Days,8-430, E/O Weekend, Rot. Holiday

    Requirements:

    + 3+ years of relevant experience

    + MA RN license

     

    EPIC experience required

     

    - Permanent MA RN lic in hand

     

    - 3 years of acute care case management experience in the last 5 years

     

    - Exp in discharge planning & utilization review

     

    - Knowledge of InterQual or similar system

     

    The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime.

    Minimum Requirements:

    + Current RN licensure in state practicing

    + At least one year of Case Management experience preferred

    + Current CPR if applicable

    + TB questionnaire, PPD or chest x-ray if applicable

    + Current Health certificate (per contract or stateregulation)

    + Must meet all federal, state and local requirements

    + Must be at least 18 years of age
