This position will be accepting applications from residents and non-residents of Colorado. This announcement is open to potential candidates who are legal residents of the United States. All legal residents of the United States are eligible to apply and compete. This position will receive a short-staffing temporary pay differential of (4%) from July 1st 2025 - June 30th 2026.
About Grand Junction Regional Center:
Grand Junction Regional Center (GJRC) is a collection of group homes that serves adult individuals with intellectual and developmental disabilities (IDD) who need additional support to stabilize and return to community residential settings. GJRC's focus is on improving outcomes for individuals with IDD as well as promoting community integration. GJRC has a total of 10 homes nestled throughout established neighborhoods of Grand Valley. These homes feature warm, comfortable living spaces and private, fenced backyards with recreation equipment.
Description of Job
The Nurse II provides comprehensive nursing care, clinical assessment, and health coordination for adults with intellectual and developmental disabilities served by the Grand Junction Regional Center. This role ensures each person’s health and safety through individualized care planning, treatment, documentation, and clinical guidance to direct care staff.
Additionally, as a Nurse II you can expect to:
+ Assess individuals’ health status, gather information from staff, and develop individualized nursing care plans.
+ Collaborate with physicians and interdisciplinary team members to support accurate diagnosis and appropriate treatment.
+ Design and update long-term and short-term nursing plans for paraprofessionals to implement.
+ Provide timely education, coaching, and direction to Licensed Psychiatric Technicians and other direct care staff.
+ Respond to acute illness, trauma, and medical emergencies using nursing judgment and approved interventions.
+ Prioritize and direct immediate responses to serious medical conditions, including when to initiate 911 or hospital transfer.
+ Complete and review medical documentation to meet state and federal regulatory requirements.
+ Investigate incidents or injuries, identify contributing factors, and propose corrective nursing interventions.
+ Oversee clinical practices such as medication administration, treatment procedures, documentation accuracy, and compliance checks.
+ Provide ongoing clinical instruction, in-service training, and support to improve staff competency.
+ Coordinate continuity of care through communication logs, follow-up plans, email updates, and interdisciplinary collaboration.
+ Communicate medical or behavioral changes to appropriate team members and ensure physician orders are routed to the correct departments.
+ Maintain licensure, CPR certification, and required competencies while participating in ongoing professional development.
+ Use electronic medical record systems, email, and agency technology to document, track, and communicate health information.
+ Travel to multiple residential and program sites to provide nursing services and support.
+ Perform additional duties as assigned to support high-quality healthcare across the agency.
This position is 100% In-Person: You are required to work at your assigned work location during your scheduled work times. Hours depend on the agency/community events and work needs. This position will work four shifts per week, 10 hours in duration. Specific schedule to be determined upon hire. Please note: Work arrangements are subject to change at any time.
Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights
Education/Licensure/Certification/Experience:
Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC). Two (2) years of professional experience in an occupational field related to the work assigned to the position
Substitutions
Appropriate education will substitute for up to two (2) years of the required experience on a year-for-year basis

Required to have a current Registered Nursing license and be in good standing with the Colorado State Board of Nursing
Conditions of Employment:
+ Full background check: CDHS employees (all direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers) and reference check required
+ This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.
+ Motor Vehicle Check, if the employee will drive a state-owned vehicle, either in an ongoing capacity while performing their normal day-to-day job duties OR on occasion for training/meetings/client visits/etc.
+ Must possess a valid, non-restricted Colorado Driver’s License or a non-restricted U.S. Driver's License for positions within 30 miles of the CO state border
+ Out-of-State Driver’s License holders must obtain a valid, non-restricted Colorado Driver’s License within 30 days of employment start date.
+ Shift work: Nursing provides 24/7 care, 365 days per year. This position may be assigned to cover any shift.
+ Position is required to use a state vehicle to transport residents.
+ TB test is required, requires Influenza and other vaccines as required by state and facility; must be 18 years or older.
+ Travel: Position must travel to remote work sites and must utilize a state vehicle, if available. If not, must utilize a personal vehicle and submit for mileage reimbursement.
+ Pre-employment Physical/Tests – Exert up to 50 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects
+ Nurses: New Compact Rule
+ Starting Jan. 2, 2024, if you have a multistate license and change your primary state of residence to another state within the Enhanced Nurse Licensure Compact (eNLC), you must apply for a license in that state within 60 days.
+ If this latter step is not completed within the allotted time frame, your previous license may expire. This puts you at risk of practicing without a license. As an example, if you are a registered nurse with a multistate license from Arizona practicing in Colorado and you move to Colorado, you have 60 days to apply to Colorado’s Board of Nursing for a new multistate license. These changes were part of the final rules passed by the Interstate Commission of Nurse Licensure Compact Administrators at its annual conference in August. The eNLC allows for mobility and creates a more robust workforce by allowing nurses to practice in Colorado with licenses from other compact states. CDHS Selection Process Explanation
Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications.
