Physical Therapist – $60/hr – Glendale, AZ

Position Overview:

We are seeking a Physical Therapist to provide evaluations and therapy services for students in grades K–8. This role includes performing assessments, implementing therapy plans, and documenting services in accordance with physician orders and individualized plans of care.

Assignment Details:

+ Location: Glendale, AZ

+ Start Date: December 18, 2025

+ End Date: End of 2025–2026 School Year

+ Schedule: Monday–Friday, 40 hours/week

+ Duration: 36 weeks

+ Pay Rate: $60/hour

+ Modality: In-person

+ Caseload: Split between 2–3 campuses (all within 15–20 minutes of district office)

Responsibilities:

+ Conduct evaluations and develop individualized therapy plans

+ Provide physical therapy services in accordance with physician orders

+ Document therapy sessions and maintain compliance with district and state guidelines

+ Collaborate with teachers and multidisciplinary teams

Requirements:

+ Current Arizona Physical Therapist license

+ Graduate of an accredited Physical Therapy program

+ One year of prior professional PT experience preferred

+ Must meet all federal, state, and local requirements

+ Current CPR, TB questionnaire, and health certificate if applicable

+ Must be at least 18 years of age
