Job Description
Good Shepherd Fairview Home is looking for a Full Time Social Worker for our beloved Long Term residents.
Position Details
Full time days, Monday- Friday: hours may fluctuate

Position Summary
The Social Worker is responsible for providing social services to resident and their families on the Long Term Care unit including: the ability to achieve their highest level of function; participate in the development pf residents' comprehensive care plans; develop policies and procedures to provide psychosocial services to residents in compliance with federal, state and local regulations and to promote person-centered care for optimal quality of care.
Essential Job Requirements
Education: Associates in Human Services or Related Field
Qualifications: Must be 18 years of age. Ability to read, write and speak the English language
Experience: One (1) year providing Human Services preferred, Experience in Long Term Care is Required
Essential Job Duties and Responsibilities
- Maintain adequate records and documentation for the Nursing Facility in compliance with state and federal regulations
- Involved in Admission process & discharge planning process
-Understanding of MDS , Careplanning and discharge planning preferred (preferred would be ideal but thy will be assisting on Cameo with DC planning)
- Prepare and participate in weekly resident care planning conferences.
- Prepare and participates and engaged in facility Quality Improvement meetings, including but not limited to Careplan Conference, Resident Council, IDT meetings, and knowledge of community resources.
-Assists with Advanced Directives
- Counsels resident and families in regard to resident advocacy related issues
-Issue ABN and NOMNCs when necessary and timely to resident and/or representative and return back to the insurance company once complete. Provide copy to HIM and Finance. Follow up with appeals process as needed
-Provide professional work ethics with peers, families, residents, while managing multiple tasks in a calm, patient and supportive manner.
-Follow Residents’ Rights policies at all times
-Consistently work cooperatively with residents, residents’ representatives, facility staff, physicians, consultants and ancillary service provider
- All other duties as assigned
Competencies & Education
-Performs competencies to keep skills up to date & current
-Completes all Healthcare Academy courses and on-going education as required
-Attends in-services as required
-Regular, reliable & dependable work attendance
