    Job Overview:

    The Clinical Supervisor will be the direct point of contact and oversee the clinical field staff and will ensure compliance and ethics of our clinical field staff.

     

    This role is a full-time role at 40 hours per week.

    Essential Functions:

    + Lead Day 2 new hire orientation for field staff in Braintree when scheduled.

    + Lead orientation training progress tracking for field staff.

    + First point of contact for field staff to escalate client concerns.

    + Work collaboratively with all field staff to create a library of localized resources to support client needs and caregiver education.

    + Supports standardized staff development across all regions.

    + Oversee the weekly CCT billing process, and escalating issues to the clinical manager as needed.

    + Manage the following processes with weekly region-specific huddles.

    + Hospitalizations

    + Weekly check ins (CCT, missed visits)

    + Discharges

    + Present in all case conference

    + MDT (Region specific- Monthly cadence)

    + Review weekly/monthly reports and escalate concerns to the manager.

    + Ensures Electronic Health Record data accurate (Important dates)

    + Documentation Submission

    + CCT compliance

    + Compliance Visits

    + Annual/Semi-Annual Compliance

    + Scheduling Review per productivity standards

    + Participate in care team management - caseload number and regions.

    + Co-lead operational team meetings (2 hours monthly - full staff)

    + Assign new clients and coverage

    + Identify areas of opportunity for new care teams

    + Support compliance and field staff efficiency through regular communications.

    + Contract and care plan at admission quality review check

    + Compliance with level changes Quality review check

    + Mailing caregiver logs, program handbooks etc. to field staff that are not frequently in the office

    + Caregiver communication to support program compliance.

    + Track MDO and Annual compliance in collaboration with Compliance department

    + 1:1 with staff monthly

    + Employee performance documentation and discussion and escalation to clinical manager with HR concerns.

    + Encourage use of Alt CG days and Alt CG credentialing.

    + Coordinate payment issues not relating to CCT or authorization lapse from field staff to account payable specialist.

    + Audit charts; intake (post assessment and post admission audits), 30 Days post admission, Semis and Annuals.

    + Foster culture of customer service and commitment to quality care.

    + Serve as a brand ambassador for VITRA reflecting our vision, mission, and values.

    + Show genuine interest and compassion for the communities we serve and commitment to the diversity of our clients and team members.

    + Perform other tasks as needed or assigned.

    + May need to support in the field as needed/assigned.

    + Community outreach as needed.

    Education, Experience and Licensure Requirements:

    + Willingness to travel within assigned region required.

    + Valid driver's license and reliable transportation required.

    + Fluency in both English and second language strongly preferred.

    + Bachelor's Degree preferred.

    + Experience supervising, managing performance, and training staff.

    + Able to work independently & willing to work in the Field as needed.

    + Previous experience in home health care or other branches of health care preferred.

    + Well versed in billing/payment/deferral/denial process.

    + Case management, behavioral health, social services and/or community health worker experience preferred.

    + Strong communication, documentation, and organizational skills are required.

    + Must be solution oriented and be able to multitask.

    + CPR Certification.

    Physical and Environmental Demands:

    + Works in a clean, well-lit environment with fluctuating temperatures in close proximity to others.

    + Requires substantial periods of repetitive work utilizing a computer, monitor, keyboard, and mouse.

    + Requires lifting and carrying equipment and supplies weighing up to 35 pounds; require pushing and pulling equipment and supplies weighing up to 35 pounds; require walking and standing; requires frequent sitting more than 75% of the workday; requires the ability to navigate stairs; requires visual acuity and manual dexterity to operate equipment.
