Job Summary:

Work with the Chronic Diseases Prevention and Self-Management Program team to enroll or lead program classes.

Job Description:

Program Recruitment (50%):

+ Recruit program participants, adhering to each program's eligibility criteria.

+ Participate in recruitment events, pitching the program information to potential participants.

+ Participate in investigator meetings and workshops.

+ Adheres to internal controls and reporting structure.

Lifestyle coach and leader (50%):

+ Conduct Chronic Diseases Prevention and Self-Management Program classes, as assigned.

+ Collect and report point-of-care blood work, biometrics, and surveys from program participants.

+ Record and report participant data and outcomes. .

+ Performs related duties as required.

Minimum Qualifications:

+ To be trained as a lifestyle coach or leader within 3 months of hire, if leading any applicable classes.

Preferred Qualifications:

+ Earning a degree in public health, healthcare administration or related discipline.

+ Bilingual (English - Spanish).

+ Experience in implementing lifestyle modification programs.

+ Experience working in academic health centers.

+ Experience in working with participants of all ages .

Salary Range:

Commensurate with experience.
