    Description:

    **Saint Francis Hospital and Medical Center** a member of Trinity Health of New England is looking for a **Department Assistant** to join our **Case Management** Team.

     

    The **Case Management Assistant** performs administrative tasks and facilitates discharges, assigns post-acute providers, and delivers discharge and change of status paperwork to our patients by working collaboratively with the Inpatient Care Coordination team to include the RN Care Coordinator and the Social Worker for each respective unit. This position requires a person who is motivated, has a positive attitude, is self-directed, and displays a professional demeanor. Accuracy and attention to detail are necessary in this position.

    What will you do

    + Pull daily reports and census from EPIC on inpatient units.

    + Maintains accurate and up-to-date documentation in the departmental records.

    + Collaborates daily with the Inpatient Care Coordinators regarding anticipated discharges and attends multidisciplinary rounds on the inpatient units as needed.

    + Assists Inpatient Care Coordinators in communicating with insurance companies via telephone and electronic portal access for authorization or denial request for patients discharging to a skilled nursing facility.

    + Will assist with skilled nursing facility and home health agency referrals via EPIC communication as needed to assist with timely patient throughput.

    + Alerts all appropriate staff members when patients have been accepted to homecare and post-acute rehab and provides transport times to staff if/when arranged.

    + Works closely with Inpatient Care Coordinators to ensure the patient’s prompt return to the nursing home when the patient is medically stable.

    + Coordinates and arranges transportation for patients returning home via Veyo and M7 transport services.

    Minimum Qualifications

    + **Education** : High School diploma or general equivalency diploma (GED) required. Associate's preferred

    + Medical Assistant Certification a plus

    + Previous Healthcare experience preferred

    + Medical terminology, computer skills, excellent communication, interpersonal and telephone skills and a friendly and professional demeanor.

    + Sensitivity to patients and families under stress of acute illness. Ability to follow the _iCare_ customer service philosophy.

    + Ability to ensure the confidentiality and integrity of protected health information

    + Demonstrated ability to manage and prioritize multiple activities while working in a fast-paced environment

    + Must be detail-oriented and have a high degree of organizational skills.

    + Demonstrated ability to set priorities and demonstrate flexibility in own assignment and work schedule.
