    Job Duties:

    + Understands and adheres to established Phoenix Home Care & Hospice policies and procedures.

    + Creates and maintains Nursing schedules.

    + Schedules Nurses according to care plans and staff availability.

    + Discerns client services required as outlined in agreements, urgent requests and care plans.

    + Enters staff and client information into database.

    + Maintains staff and client database

    Requirements

    + Troubleshooting problems and providing resolution

    + Ability to handle a multi-line phone system

    + Highly Organized & Detail Oriented

    + Previous Care Coordination a plus!
