    Heritage Ministries is in growth mode and currently looking for a Social Worker to join our team! A Social Worker is responsible to act as a liaison for the residents’ social interests and to plan, organize, and attend to the psychosocial needs of the residents in accordance with current federal, state and local standards.     Responsibilities:

    + Responsible for maintaining documentation of social work activities as required by state and federal rules and regulations.

    + Participates in ongoing evaluations and quality assurance projects.

    + May assist with the development of departmental policies and procedures to ensure the highest quality of care to the residents

    + Helps determine any unmet psychosocial needs of the residents through regular contact with residents and/or the family and through staff referrals. Arranges help as needed through internal and external sources.

    Qualifications:

    + Bachelor degree in a related field or minimum 3 years experience in the social work field.

    + Education or experience in long term care preferred.

    + Ability to lift 60 pounds if necessary with the ability to sit, walk, bend, and twist. Ability to engage in repetitive motions for extended times required.
